Home Health Physical Therapist – Advanced Home Health of Arizona
Location: Glendale, Arizona
Schedule: Full-Time | Monday–Friday | 8:30 a.m. – 5:00 p.m.
About the Role
Advanced Home Health of Arizona is seeking a compassionate and skilled Physical Therapist (PT) to join our Glendale-area home health team. This full-time position offers the opportunity to make a meaningful difference in patients’ lives by helping them regain strength, mobility, and independence in the comfort of their own homes.
You’ll work as part of a supportive, interdisciplinary team dedicated to delivering exceptional, patient-centered care throughout the community.
Key Responsibilities
- Evaluate and develop individualized treatment plans for home health patients.
- Provide physical therapy interventions to restore function, relieve pain, and prevent disability.
- Educate patients and caregivers on exercises, safety, and mobility strategies.
- Collaborate with nurses, physicians, and other healthcare professionals to ensure coordinated care.
- Maintain accurate and timely documentation in accordance with regulatory and agency standards.
Requirements
- Active Arizona Physical Therapist License (required)
- Minimum of 1 year of clinical experience preferred (home health experience a plus)
- Strong communication and critical thinking skills
- Compassionate, patient-focused approach to care
- Reliable transportation and willingness to travel locally in the Glendale area
