This position plays a vital role in facilitating the referral, admissions, and intake process for new clients. Serves as a critical liaison between clients, clinical teams, referral sources, and administrative departments to ensure a smooth, timely, and coordinated transition into care. Oversees client flow between programs, working closely with program teams to optimize occupancy, reduce bottlenecks, and support continuity of care. Ensures that all required documentation is collected and verified, bed assignments are made appropriately, and transitions between units are handled efficiently and in compliance with regulatory standards.I. Major Responsibilities:
1. Engages in community outreach and education to key stakeholders to improve awareness of eligibility criteria and admissions process, solicits feedback, and participates within community workgroups as assigned.
2. Develops and implements plans to increase referrals, reduce barriers to admission, and facilitate transitions to lower levels of care.
3. Escalates referral review to necessary to medical and clinical personnel for additional review as needed.
4. Facilitates daily internal and external communication to provide information about bed availability, waitlists, and to coordinate access.
5. Ensures that all aspects of programmatic referral and admissions workflows are implemented. Collaborate with program management to improve these workflows.
6. Ensures timely response to and documentation of all referrals.
7. Supports admissions workflow - assists with referral call response, coordinates admission appointments, provides information to clients; ensures communication with sending and receiving program, ensures all required documentation is complete.
8. Initiates quality improvement measures to improve referral management, efficiency, and responsiveness.
9. Works collaboratively with program management to review referrals, responsiveness, and quality of communication
10. Compiles summary reports of referrals and admission data, identifies trends, strengths, or areas for improvement.

License/Certification/Education:

1. Bachelor’s degree.
2. Must pass CORI background check.
3. Driving is not a requirement.

Experience/Skills:
Required:
1. Minimum two (2) years of previous experience in human services, health care, or customer service.
2. Must be able to use a computer for basic tasks, including sending emails and using web-based applications like online forms or scheduling tools.
Preferred:
1. Previous experience using an electronic health record.
Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
III. Physical Demands and Environmental Conditions:
Work is considered sedentary. Position requires work indoors in a normal office environment.
