Admission/Visit RN
Rotating Schedule
Week 1: Monday/Wednesday/Friday 8:00 a.m. - 8:00 p.m.
Week 2: Tuesday/Thursday/Saturday 8:00 a.m. - 8:00 p.m.
No On-Call
Coverage Area: Porter, La Porte, Starke, Jasper, Newton and Lake Counties

Job Duties:
Demonstrates comprehensive knowledge and skill in current nursing practice.
Exhibits knowledge of the concept of an interdisciplinary team and a working knowledge of the development of plans for patient care and the ability to participate in them as well as using the case management approach.
Functions independently and appropriately and demonstrates the ability to deal effectively with the demands of the job.
Summary-
The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching, and supervising the health care of hospice patients in their homes. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.
- Possesses understanding of Hospice program and benefits.
- Obtains orders for and procures immediate medications and needed DME/supplies.
- Performs physical assessment of patient's condition, safety, knowledge, pain, and symptoms.
- Initiates Plan of Care
- Instructs on disease understanding, progression, and immediate interventions.
- Documents assessment, and current medications and initiates appropriate care plans.
Requirements
Qualifications/Education:
1. Licensure: Must have and maintain a license as a registered professional nurse by the state of Indiana. Minimum of Associate degree in Nursing from an accredited program.
2. The diploma must be from an approved school of professional nursing.
3. A minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care is preferred.
4. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Must be sensitive to issues of loss and grief.
5. Must maintain current CPR certification.
6. Must hold and maintain a valid driver’s license and auto insurance.
7. TB status must be current.
