LOCATION: Dublin, GA
The Community Service Board of Middle Georgia's CSU is looking for a Discharge Planner. The Discharge Planner will work hand in hand with the Charge Nurse and medical staff to ensure that admissions and discharges are performed in a timely manner. The Discharge Planner will also work with patients to ensure that care and services are properly provided.
Responsibilities of the Discharge Planner:
- Assist with admissions if necessary regarding searches and works with the individuals throughout their stay regarding their desired discharge plans and specific aftercare needs.
- Recognize individual needs and assist them in obtaining appropriate services.
- Assures that all information specified in the treatment plan have been completed and an adequate progress note to correspond with the treatment given is placed in the individual’s medical record.
- Arrange follow up appointments for those being discharged and document appropriately when plans are made. Also make sure individual is aware of appointments and plans; even after discharge as needed.
- Make sure discharge summary/plan is on all individual charts with signatures, make sure this is reviewed and in final status.
- Attend treatment team every Monday, Wednesday, and Friday at 7:30 AM. Communicate with the provider and other interdisciplinary members during treatment team about discharge plans and discuss the need for LOC.
- Keep a monthly log book of discharge call backs to ensure continuity of care.
- Assist in transporting individuals home or to their discharge placement if/when needed.
- As necessary/needed, provide watchful oversight and supervision to the individuals.
- Be sure to review any discharges that may have taken place prior to appointments made and make sure an appointment is documented and the individual is aware.
- Assist with submission of LOC and order for services; and adult suicide risk alerts.

- High School Diploma/GED Equivalent
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
- Ability to organize, prioritize and meet deadlines accordingly
