Plans, develop, and implement approved evidence based intervention (EBI) prevention and health promotion programs for the 13 counties in the Alamo Area Council of Governments (AACOG) service region. Provides consultative services and technical assistance to program staff, governmental agencies, community organizations, and the public. Ensures that approved health and wellness EBI programs adhere to national standards. Recruits, trains, monitors, and advises health and wellness leaders/coaches/facilitators to ensure adherence to national standards. Develops, facilitates, and supports working partnerships with other area agencies on aging, state and local agencies, health care providers, and community based organizations to increase the number of volunteer leaders/coaches/facilitators, host sites, and contracted vendors.
This is a responsible, non-supervisory position. The responsibilities of this job include planning, developing, and implementing approved EBI programs; coordinating and training program leaders/coaches/facilitators; providing consultative services and technical assistance; maintaining program files and documents; preparing and conducting educational presentations; attending health fairs and community events; and working with other agencies to promote related programs and services. Builds and maintains reference files on sources of public health data and information. Participates in program planning and development, including enhancing program participation and engagement through virtual and in person service delivery. Prepares and conducts public presentations and provides education and advocacy surrounding EBI programs. Coordinates and participates in activities that further the development of EBI programs with an emphasis on health, safety, and wellness. Complete daily, weekly, monthly, and yearly reports, whether internal or external, such as state reports in the appropriate database, reports to track volunteer hours, Area Judges report, and Alamo and Bexar Senior Advisory Committee reports. Works with program manager/director on tracking budget expenses. Acts as liaison to the Alamo and Bexar Senior Advisory Committees to increase community engagement and collaboration. Adheres to quality assurance plans, policies, and procedures per internal and external agencies. Properly documents time in ADP or its equivalent and documents all work product.
Acts as a liaison to community based partner organizations relating to EBI health and wellness programs, providing them with advice, technical assistance and program resources and materials for the development, expansion, and maintenance of their programs. Maintains files which support the programs, including grant awards, performance reports, correspondence, budgets, billing, report submissions, and any other program related documentation. Responsible for ensuring confidentiality of training, participant, leader/coach/facilitator records in accordance with EBI and Health and Human Services Commission (HHSC) program standards and Health Insurance Portability and Accountability Act requirements (HIPAA). Works with internal agency groups to secure funding and to carry out the provisions of any grant received. Serves as the primary participant in grant applications to further the health and wellness activities for the program. Provides for the recruitment, training, quality, and retention of volunteer leaders/coaches/facilitators. Develops, implements, and evaluates learning curriculum and workshops for health and wellness education in the community. Develops and disseminates materials for regional use. Must be able to travel within the 13 counties in the AACOG service region. Performs other job related duties as assigned.
Desired Education/Job Experience: Some college preferred; Social Sciences or Health Sciences or a related field; Two (2) years of related experience or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities. Bilingual in English and Spanish.
Other Qualifications: Must become a trainer for approved EBI programs already implemented by the agency within six months of hire date (dependent upon funding and available training), or within three months of implementation of a new EBI program (dependent upon funding and available training). Must have a valid Texas driver's license and reliable means of transportation.
This position is a Pay Grade 10, with a starting hourly rate of $20.10. This is a non-exempt position. Open until Filled. Only electronic online applications will be accepted.
