Description

COMMUNITY HOSPICE is seeking an experienced PRN CRNP
The primary objective of the Nurse Practitioner is to assist in performing the Face-to-Face encounters to determine continued eligibility for hospice. This position will collaborate and cooperate with the Medical Director as well as the RN Case Manager.
This position will consult with team members for clinical information relating to the patient/family. Responsible for reviewing patient records and documenting the clinical findings.


Requirements

  • Registered Nurse licensed to practice as a Certified Registered Nurse Practitioner in the State of Alabama.
  • Comprehensive knowledge and well-developed skills in: Medicine and Pharmacology, Pain and Symptom Control and Psychology of loss.
  • Valid Alabama Drivers License and automobile insurance.
  • Understanding and acceptance of Community Hospice of South Alabama principles of care.
