Scope of Position: The Health Homes Care Coordinator Supervisor and Quality Specialist focuses on achieving the six core care management functions within the designated health home program. They provide direct supervision to care coordinators to ensure the efficient and effective delivery of care management services. This includes overseeing supervisees to ensure that all members achieve their goals as outlined in comprehensive patient/family-focused care plans.
Essential Position Functions:
The Care Coordinator Supervisor will supervise day-to-day activities of the Care Coordination team.
Participate in group and individual supervision directed by the Program Director. Lead team meetings with Care Coordinators to implement goals, plans of care, assessments, HARP, and core services, and ensure timely documentation.
Support training and team building for staff development.
Utilize electronic databases required by Health Homes. Manage a caseload, providing expertise and stabilization for specific individuals, including AOT consumers transitioning to Care Coordination.
Address client issues during their enrollment in the Health Home program.
Provide client-centered, goal-oriented supervision focused on data-driven outcomes.
Monitor client charts and documentation for compliance with regulatory guidelines.
Conduct one-to-one and group clinical supervision sessions for Care Coordinators.
Support case conferencing and coordinate incident reporting procedures.
Alert care coordinators to important follow-up items as needed.
Complete required weekly and monthly reports.
Demonstrate excellent interpersonal, written, and verbal communication skills to support staff development and service implementation.
Undertake additional tasks and initiatives assigned by the Program Director or Senior Director.
Requirements
Required: Master's degree.
Preferred: Background in mental health and group experience.
Excellent proficiency in both verbal and written English and Spanish.
Familiarity with AWARDS system and internet use is preferred.
Willingness to engage in trauma-informed training and interventions
