About the Department

Definition:  Under general supervision from the assigned Health and Human Services department leader, performs professional social services casework primarily designated to support individual case needs and treatment goals.  This position ensures that clients have access to appropriate treatment, community resources and support systems to support recovery and stability. This position collaborates with interdisciplinary teams and other community stakeholders. This role also facilitates discharge planning congruent with therapeutic and Social Determinant of Health Goals (SDOH), to ensure client continuity of care. The Social Worker also provides direct client care to include: Social Work Coordination of Care, Psychoeducational Groups and/or contributing to the development of client goals and needs with the assigned primary counselor. This job class is treated as FLSA Non-Exempt.

Position Duties

1. Provides follow up and support services to individuals and their families: 

  • Facilitates referrals to housing, employment support, substance abuse/mental health treatment, medical providers, community support agencies to meet client needs and support goals. 

2. Documentation and Compliance: 
  • Maintain accurate, timely and confidential records in compliance with HIPAA and Community Standards. 
  • Participates in quality improvement initiatives to improve client outcomes

3. Direct Service: 
  • Provides consultation and support to clinical staff regarding SDOH and resource access. Scheduling and Facilitating Adult and Recovery Team (ART) meetings to support client needs with interdisciplinary team
  • Participates in and/or develops client transitions plans to ensure continuity of care ie: (Discharge Plans, Reentry Plans, and Transition/Transfer Plans). 
  • Provide services to support client needs such as: Psychoeducational Groups/Individual services, Client staffing’s, facilitate monthly or as needed ART meetings, Social Work/Case Management, 
  • Meeting department productivity expectations of required billable services. 

4. Administrative Tasks: 
  • Participates in training opportunities that will enhance practice of Social Work in assigned department.  
  • Attends professional development meetings and training as directed by immediate supervisor.  
  • Helps maintain an environment within the HHS Department that encourages teamwork, interdependence and ethical behavior.
  • Performs other job-related duties as assigned to maintain and enhance departmental and program operations.

Knowledge, Skills, Abilities, and Other Characteristics:

  • Knowledge of the history, culture, laws, rules, customs, and traditions of the Salt River Pima-Maricopa Indian Community.
  • Knowledge of the principles and practices regarding behavioral health and recovery. 
  • Knowledge of ASAM and Behavioral Health Assessment and Treatment best practices.
  • Knowledge of social work principles and concepts.
  • Knowledge of social work methods and techniques, including group and individual processes, crisis intervention and referrals.
  • Knowledge of local resources to support client needs. 
  • Knowledge of confidentiality laws surrounding behavioral health and tribal standards. 

  • Skill of ability to approach client with respect, openness and a willingness to learn from the community. 
  • Skill with integrating cultural practices in care plans
  • Skill with strong verbal and written communication skills
  • Skill with ability to provide support through a trauma informed lens. 
  • Skill with, scheduling and time management.
  • Skill establishing and maintaining effective working relationships with other social work professionals, SRPMIC co-workers, Community officials and clients.
  • Skill evaluating customer needs and utilizing SRPMIC resources or other appropriate referrals to meet client needs.
  • Skill dealing with sensitive, personal issues and maintaining appropriate professional, confidential relationships.

  • Ability to maintain professionalism consistent with the mission, vision and values of the Salt River Pima Maricopa Indian Community
  • Ability to work with client caseload and individualize client needs based on client assessment and interaction.  
  • Ability to establish and maintain accurate record-keeping systems and procedures including computerized record-keeping.
  • Ability to work under pressure with multiple task deadlines.
  • Ability to use personal computers, related software and other standard office equipment such as facsimile, photocopier and calculator.
  • Ability to drive SRPMIC vehicles.

Minimum Qualifications

Education and Experience:  A Bachelor’s degree from an accredited college or university in Social Work or a closely related field, AND experience equivalent to one to a minimum of one (1) full-time professional level work in the social services field.

Equivalency: Any equivalent combination of education and/or experience that would allow the candidate to satisfactorily perform the duties of this position will be considered.

Underfill Eligibility: An enrolled Community Member whom closely qualifies for the minimum qualifications for a position may be considered for employment under SRPMIC Policy 2-19, Underfill.

Other Qualifications

  • May be required to work beyond normal work hours including nights, weekends and holidays.
  • Due to the confidential and sensitive nature of this position, successful completion of an extensive background investigation is required.
  • May be required to travel using own vehicle.
  • Will be required to make home visits and deliver services in the home.
