DEFINITION:

The Care Worker is responsible for providing direct social services to Hospice patients and their families. The Care Worker provides the initial emotional, spiritual, and psychosocial assessments, bereavement services and community education, outreach, and referrals. 

 

EDUCATION:

Minimum of five years of formal education. Masters Degree in Human Services with training or experience.  Training will include information regarding age appropriate standards for casework with individuals of all ages.

 

REGULATORY REQUIREMENTS (IF APPLICABLE):

Licensure or certification is encouraged when applicable.

 

MINIMUM QUALIFICATIONS:

Bachelor’s Degree in Psychology from an accredited school, and will work under the supervision of a person with a Master’s in Social Work.

 

PREFERRED QUALIFICATIONS:

Two years of medical social work or case management in a health care setting is preferred.
