Description
JOB SUMMARY:
Responsible for providing medical case management focused on maintaining Ryan White patient in systems of primary medical care to improve health outcomes.
DUTIES AND RESPONSIBILITIES:
- Completes client intakes, needs assessments, and care plans upon initial contact with new clients.
 - Maintains regular contact with all clients and completes Needs Assessments and Care Plans with clients, with the goal of client self-sufficiency.
 - Coordinates and monitors medical appointments, medical adherence and drug treatment adherence.
 - Provides health education and risk reduction services to include treatment adherence. Provides resource contact information, coordinates and completes referrals for services needed by clients.
 - Coordinates with community partner agencies to ensure referred services are being provided in a timely manner.
 - Documents all client contacts daily in client EMR chart and Take Charge Texas (TCT), and/or other Ryan White database, along with notes of Care Plan.
 - Evaluates patient’s Ryan White Eligibility to include Dental Referrals received by other agencies for Part D.
 - Makes information regarding services offered available to the public; uses necessary strategies to identify potential patients.
 - Explains program services, policies and procedures to patients and staff.
 - Coordinates and assists with Medical Outreach events in the community.
 - Abides by infection control and hazardous materials policies and procedures as dictated in the Centro Del Barrio Infection Control Manual.
 - Participates in the agency’s Quality Improvement Program; to include assisting in audit preparation.
 - Abides by all safety policies as dictated in the Employee Safety Manual;
 - Performs other duties as required by the position.
 
Requirements
QUALIFICATIONS:
- Education: Minimum of a Bachelor’s Degree in Social Work or related field.
 - Experience: Two years’ experience providing medical case management in a health care setting.
 - Other: Good understanding of working with indigent populations.
 - Must have own transportation and auto insurance. Current Texas State licensure.
 
PHYSICAL REQUIREMENTS:
- Must be able to perform the essential functions of the position.
 - Must be able to meet the attendance requirements of the position.
 - Must not pose a threat to the health or safety of other individuals in the workplace.
 - Physical ability to lift up to 20 pounds unassisted.
 - Must be able to deliver excellent customer service, externally and internally.
