Job Type: Per Diem
Location: Oceanside/Long Beach region
Pay: $65-$85 per visit (can choose either W2 or 1099)
What You'll Be Doing:
- Provide one-on-one occupational therapy in the home setting
Develop and update individualized plans of care with patients, families, and physicians - Document visits in our quick EMR (no OASIS required)
- Communicate progress and discharge planning with patients and families
- Treat patients primarily within your designated geographic area

Requirements
- NYS Occupational Therapy License (required)
- Homecare experience preferred (mentorship & training provided if new to home health)
- Compassionate, patient-centered care and strong communication skills
- Ability to travel to and from the homes of your patients throughout your given territory.
