Position Summary

This position is responsible for supporting all aspects of the HealthySteps program operations and training for the expansion of the program at additional pediatric clinic locations at Children's MN. The HealthySteps Program Coordinator performs all administrative tasks for the program and will collaborate with the foundation grant writers for the purpose of creating and submitting grant applications. This position will report to the clinic supervisor and will work closely with the HealthySteps Specialist(s) who provide consultations and care during paired primary care visits.

The HS Program Coordinator will interface directly with patients and their families as well as medical team members to provide operational and administrative support to the program. The HS Program Coordinator will need to be able to meet tight deadlines and manage several projects simultaneously, respond rapidly to conflicting priorities and handles tasks with a high degree of accuracy. Familiarity with the primary care medical settings for low-income families including work flows, EMR systems, and team-based care is preferred. 

The HS Program Coordinator provides registration assistance for patients and helps patients access social service resources.

License/Certification/Registration required? No

Education:
*Bachelor's degree in child development, psychology, social work, public health, early childhood education or related field and / or equivalent experience / training required
*Master's degree in child development, psychology, social work, public health, early childhood education or related field preferred

Experience:
*Exposure to principles of trauma-informed care as they relate to an institution
*Demonstrated competency in empathetic listening, de-escalation techniques and motivational interviewing
*Experience / familiarity with an Electronic Medical Record
*Experience in a healthcare environment preferred

Knowledge/Skills/Abilities:
*Ambulatory care clinic practice management including patient care workflows and processes
*Knowledge of community-based resources for social determinants of health preferred
*Proficient in Microsoft Word and Microsoft Excel
*Strong communication skills
*Strong organization skills
*Skills in data management
*Demonstrates strong interpersonal skills with diverse staff and patients
*Ability to effectively manage time and prioritize tasks

The posted salary represents a market competitive range based on salary survey benchmark data for similar roles in the local or national market. When determining individual pay rates, we carefully consider a wide range of factors including but not limited to market indicators for the specific role, the skills, education, training, credentials and experience of the candidate, internal equity and organizational needs.

In addition to your salary, this position may be eligible for medical, dental, vision, retirement, and other fringe benefits. Positions that require night, weekend or on-call work may be eligible for shift differentials or premium pay.
