Position Title: Transitional Case Manager
Supervisor: Case Manager Supervisor/Director of Community Integration
Status: Full-time, hourly, non-exempt, starting at $22.25/hour

Job Description:
The Transitional Case Manager is a member of the Community Integration Services Team. This person is responsible for the support of clients beyond the 90 day resettlement period. The Transitional Case Manager works closely with staff from other departments to receive referrals and identify needs of clients, helping hem work towards self-sufficiency.

Primary Responsibilities:
- Maintain caseload of clients who require any variety of post-resettlement services including assistance with housing, public assistance, Medicaid or other issues
- Receive referrals from JERS legal, education and employment departments as well as Walk In clients who are program eligible
- Ability to advocate for cleints, while at same time empowering them
- Complete intake process as needed with newly referred clients
- Create assessment for each new client document all services
- Participate in walk in client process
- Maintain detailed, timely documentation of work through written case notes
- Conduct screenings for WNYICC program
- May provide limited resettlement services
- Other duties as assigned.
Qualifications:
- Bachelor’s degree in Social Work or equivalent degree in a related field, or combination of experience and education
- At least one year of experience working in a human services field, providing direct services such as case worker, social worker, employment counselor, etc.
- Ability communicate effectively in writing and verbally in English
- Ability to work with diverse client groups in a variety of settings
- A passion for resolving challenging problems and thinking outside of the box
- Strong ability to work as a member of a team
- Valid driver’s license and access to an insured vehicle
