About Harbor Health ESP
Harbor Health ESP is a state-certified PACE program based in Brockton and Mattapan, MA, providing comprehensive healthcare, transportation and social services for more than 500 frail elders who continue to live with dignity and independence in the community. Our team of healthcare professionals address the unique clinical and social needs of each elderly person in the program.
Our current web presence includes a main site and several satellite websites and platforms. These have grown organically and now need to be aligned under a unified, compelling, and sustainable design.

Project Overview
Harbor Health ESP seeks a design partner to redesign our main website and provide a strategy to consolidate and align our suite of sites. The redesigned platform should be visually appealing, accessible, and easy to update by staff. It should highlight our impact, provide interactive features, and offer a clear user journey for participants, caregivers, referral sources, and providers.
