About the Role:

Key Responsibilities:
- Conduct initial hospice assessments in patients’ homes or care settings
- Develop personalized care plans alongside the interdisciplinary team
- Provide education, comfort, and support to patients and their loved ones
- Ensure smooth transitions into hospice care with complete, timely documentation
- Advocate for patient preferences while collaborating with providers and caregivers
- Support families emotionally throughout the hospice journey
- Participate in team meetings and continuing education
- Uphold compliance standards, HIPAA regulations, and ethical care practices
Requirements
What We’re Looking For:
- Licensed RN in the state of Texas
- At least 1–2 years of nursing experience (hospice or home health preferred)
- Strong assessment and critical thinking skills
- Empathetic, calm presence during challenging family moments
- Comfortable traveling to homes, facilities, and community sites
- Tech-savvy with documentation systems (iPad/iPhone use)
- Bilingual candidates and diverse clinical backgrounds welcome
