Pay: $19.00 - $23.00 per hour

Schedule will vary between the hours of 7am-10pm.

To provide peer support to pregnant and postpartum people who have been identified as having risk for substance misuse. Service provision will focus on connecting families to needed community resources including home visitation programs.

Specific Duties:

· To provide an individualized assessment of identified families eligible for Peer Navigator Pilot

· Assist families in connecting to community resources.

· Provide families with a warm handoff to the home visiting service that best fits the needs of the family.

· To provide scheduled follow-up for families to identify any additional needed support.

· To assess for and provide facilitation of the completion of a plan of safe care on every identified family.

· To participate in staff training and development.

· To attend monthly Peer Navigator Group Mental Health Consultation

· To advocate on behalf of persons with behavioral health problems to protect the family's rights and to assist in reducing associated stigma.

· Any other duties as assigned by hiring agency

Employment Qualifications:

Education: High School Diploma or GED required. Certification to become a Peer Recovery Coach will be expected upon hire.

Experience: Past or present recipient of mental health or substance abuse services with at least 3 years of long-term uninterrupted sobriety. Past recipient of home visiting services preferred.

Skills: Ability to work well with others. Familiarity with the Genesee County community resources.
