• Advocacy & Education: Empower and educate clients and families with information about mental health resources, helping them engage in their own care and navigate the healthcare system effectively.

• Virtual & In-Person Support: Effectively coordinate care remotely while engaging in direct client interactions as needed. Must be able to work on an on-call basis when necessary

• Daily Staff Collaboration: Maintain strong communication with healthcare teams to align care strategies and address client needs.

• End-of-Day Reporting: Provide structured updates to relevant departments beyond case notes.

• Client Engagement & Trust Building: Foster meaningful relationships with clients to enhance treatment outcomes and provide emotional support.

• Clinical Documentation: Accurately record patient progress, changes and all care information concisely in electronic health records (EHR) systems while ensuring compliance with healthcare regulations and following timeline requirements.

• Problem-Solving & Follow-Up: Address challenges in client care, proactively identifying solutions and ensuring timely responses.

• Resource Navigation: Connect clients with essential wraparound support services, including housing assistance and employment programs.

• Transition Management: Guide clients through changes in care settings while maintaining continuity of care.
