Description

Basic Function/Overall Responsibilities: To provide support and safe transportation for clients of the agency to and from medical/ clinical, educational, home visits and dental appointments in accordance with the agency's vehicle policy.


Principal Assigned Responsibilities:

  1. Coordinate all medical appointments and prepare necessary paperwork for such appointments.
  2. Transport clients to and from medical/dental appointments to include out of town appointments. Provide transportation to educational and home visits as scheduled.
  3. Coordinate follow-up appointments as necessary, collect provider's notes; make necessary copies to give agency providers and nurses assigned to the client.
  4. Provide support supervision to youth with medical needs.
  5. Organize and keep appointments logbook up-to-date.
  6. Pick up medical equipment for client's post appointments to include but not limited to eyeglasses, hearing aids, etc.
  7. Complete all required agency trainings.
  8. Maintain TCI certification.
  9. Maintain Med Certification.
  10. Other tasks as requested by supervisor(s).

Qualifications


Education and Experience Requirements:

 l.    Clean driving abstract. Must have three years of driving experience. 
2.    High school diploma or GED.
3.    Preferred experience with children with behavioral/psychiatric issues.

5.    Understanding of appropriate infection control practices, including appropriate hand washing procedures.
6.    Follows mandated repotting requirements regarding child abuse and neglect.
7.    Asks age, developmentally appropriated questions, and actively listens to children's responses.
8.    Valid New York State driving license.
