Care Manager – Job Overview
Care Managers play a vital role in supporting clients as they work toward independence and wellness. You’ll develop and regularly update person-centered service plans, ensuring each client’s needs and preferences are met.
Key Responsibilities:
- Create, monitor, and update individualized service plans
- Educate clients on medication management, healthy lifestyles, self-care, and psychosocial needs
- Coordinate with clients, providers, health plans, and care teams to ensure services are delivered effectively
- Link clients to necessary treatments such as substance use or trauma therapy
- Facilitate discharge planning and smooth transitions back into the community
- Provide brief, supportive counseling and assist with housing searches
- Help clients set up new living arrangements, including transportation, household items, and benefit applications
- Attend and support clients during medical and psychiatric appointments
- Conduct home visits and post-discharge follow-ups
- Encourage the development of independent living skills
- Stay current with professional development and required certifications

Requirements
The requirements listed below represent the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
EDUCATION, SKILLS, and/or EXPERIENCE:
- Master's degree in social work, psychology, counseling and related Field.
- Three years of experience working with clients who have serious and persistent mental illness, substance use disorders and/or medically complex needs.
- Bilingual in Spanish & English preferred
- LCSW or LCPC preferred but not required.

Compensation & Benefits:
- Base Salary: $60,000 per year + additional quarterly productivity bonuses
- Paid Time Off: 11 Vacation days, 12 Holidays days, 3 sick days, 3 personal days
