Community Connector
 
Buckinghamshire Mind is looking for a Community Connector to join our team, working in the Berkshire area of our service.
The Community Connector is responsible for supporting patients to access a wide range of community services and resources including social care, housing, family, debt and employment counselling that support maintaining good health and wellbeing.
  • Contract Type: Permanent 
  • Location: Windsor, Ascot and Maidenhead
  • Hours: 37 hours per week Mon-Fri
  • Salary: ÂŁ24,097.50

We promote wellbeing and recovery; we prevent mental ill health; we offer talking therapies and we provide support in times of crisis.
 
Key Responsibilities:
Working as part of a multi-disciplinary team located in primary care:
  • Collaborate on Assessments & Goal Setting – Attend initial client assessments with the Mental Health Practitioner, support risk assessments, and assist patients in identifying needs and goals.
  • Empower Patients & Promote Independence – Provide self-management tools, support personal support plans, and encourage independence using a strength-based approach.
  • Facilitate Integrated Support – Build strong relationships with agencies, GPs, and healthcare professionals to ensure coordinated care and seamless recovery support.
  • Ensure Continuity of Care – Help patients access essential services like Safe Havens, CMHT, and local well-being programs to support recovery and long-term well-being.
 
The ideal candidate will be
  • Qualified & Experienced – Holds a relevant degree, diploma, or equivalent experience, with a background in support work or community engagement.
  • Self-Sufficient & Organised – Manages workload independently, maintains accurate records, and effectively utilizes IT-based social networking.
  • Empathetic & Culturally Aware – Understands mental health challenges (personal or professional experience) and values diversity in local communities.
  • Strong Communication & Collaboration – Collaborates effectively with diverse teams, professionals, and individuals to deliver inclusive, impactful support, while actively promoting the service to both professionals and community settings.
  • Flexible - Ability to work flexibly and adapt to the needs of the role (can include evening and weekend working). 
  • Driver with access to a vehicle - As this role is predominantly based in the community supporting individuals 1:1 in the community, with travel between several locations, a full driving licence is preferred and access to a vehicle for use during work hours. Alternative modes of transport may also be considered.
