Continuing Care Specialist

Position Overview:

Centered Health is seeking a highly organized, people-focused, and proactive professional to join our team in the role of Continuing Care Specialist. This remote contractor position is responsible for cultivating and maintaining strong, trust-based relationships with external treatment providers who have referred clients to Centered Health, as well as fostering ongoing connections with the professionals and programs to whom we have referred clients post-discharge.
 
 Your work will directly support continuity of care, treatment provider satisfaction, and long-term collaboration that benefits both our clients and our provider network.

Key Responsibilities:

Treatment Provider Relationship Management (Discharged Clients):

• Maintain active, professional relationships with external treatment providers (e.g., therapists, psychiatrists, educational consultants, hospitals, and school counselors).
 • Conduct routine post-discharge check-ins with providers to whom we have referred clients, ensuring the referred clients are progressing well and evaluating whether the provider remains an appropriate resource for future referrals.
 • Aggressively pursue new referrals from providers who have previously received clients from Centered Health, leveraging successful outcomes and relationship-building to generate ongoing referral streams.
 • On average, Centered Health discharges approximately 18 clients per month, representing consistent opportunities to engage providers and strengthen our network.
 • Work closely with the Director of Admissions to identify and develop new treatment provider relationship opportunities, with a focus on long-term strategic growth.
 • Document all treatment provider communication in Salesforce (or designated CRM), tracking engagement, satisfaction, and relationship status.
 • Collaborate with clinical and admissions teams to gather and communicate post-discharge information in a HIPAA-compliant manner.
 • Organize regular touchpoints (Teams/Zoom check-ins, follow-up emails, thank-you notes) at key intervals (7-day, 30-day, 90-day).
 • Track treatment provider satisfaction and assist with reporting for quality improvement and business development purposes.

Outside Provider Coordination (Active Clients):

• Identify and maintain communication with current client providers (e.g., outpatient therapists, psychiatrists, schools, case managers).
 • Ensure all communications are timely, HIPAA-compliant, and clearly documented.
 • Facilitate consistent updates between Centered Health’s internal teams and external providers to support integrated treatment planning.

Qualifications:

• Strong interpersonal skills and a warm, professional communication style.
 • Excellent written and verbal communication skills in English; native English speaker required.
 • Self-motivated with the ability to work independently in a fully remote environment.
 • Comfortable using electronic health records and CRM platforms (e.g., Salesforce, Kipu).
 • Knowledge of HIPAA regulations and healthcare communication practices.
 • Ability to multitask and manage multiple provider relationships with high attention to detail.
 • Ability to produce and submit weekly, monthly, and quarterly reports to the Director of Admissions.

This role is work from home. Part-time: 20 hours a week, M-F, Flexible hours. Location is in the state of Arizona.
