Hours: Full-time (37.5 hours per week), Monday through Friday, 8:30 a.m. – 5:00 p.m. 

Location: Amherst VA, 24521

Compensation dependent on experience 

 

Position Summary

The Case Manager 1 is an entry-level position responsible for providing case management with individuals and their families to establish appropriate linkages, referrals, and coordination of care, and to provide regular follow-up on needed services and supports under the supervision of a qualified Case Management supervisor. The Case Manager 1 will identify and assess client needs and evaluate, coordinate, and ensure necessary services are provided.

Required Education and Experience

  • Bachelor’s degree from an accredited college or university in an applicable human services field (including social work, counseling, behavioral science, psychology, or a related field).
  • No professional experience required – less than one year of human services experience. Prefer some relevant experience.

Related Knowledge, Skills, and Abilities

  • Knowledge of the nature of serious mental illness, developmental disabilities, and substance use, depending on the population served.
  • Ability to observe and report accurately on the functioning of individuals and families.
  • Ability to engage and sustain ongoing relationships with individuals receiving services.
  • Ability to learn and acquire the knowledge and skills necessary to perform all Case Management services to clients.
  • Ability to handle confidential information appropriately.
  • Must possess time management skills.
  • Ability to effectively use active listening skills. Ability to listen to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
  • Ability to work as a team member, maintaining effective inter-agency and intra-agency working relationships.
  • Ability to work independently, performing position duties under close supervision initially and then general supervision going forward.
  • Ability to document in an electronic medical record system.
  • Ability to be persistent and remain objective.
  • Proficient with MS Office
  • Ability to organize work efficiently and effectively.
  • Ability to use effective problem-solving techniques in daily work.
  • Ability to be culturally sensitive in the work environment.
  • Must demonstrate critical thinking in the work environment.
  • Ability to handle daily stressful situations and a fast-paced work environment.
  • Demonstrate flexibility in the work environment.

  • Knowledge, understanding, and commitment to professional ethics.
  • Ability to make home visits and to travel.
  • Strong verbal and written communication skills.
  • Must possess a valid driver’s license. May be called upon to travel to any Horizon worksite, transport clients, and provide services to clients at other locations. Regular attendance at training and Horizon meetings may require travel to a variety of Horizon sites.
  • Ability to participate in and successfully pass behavioral intervention training, which includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
  • Willing and available to work modified schedules (weekends, holidays, etc.) as required.
  • Ability to pass all required training modules.
