Job Summary: 

The Family Medicine Nurse Practitioner at Greater Baden Medical Services (GBMS) is responsible for providing direct patient care and care consultation for a panel of patients as well as patients at GBMS. This position requires strong family medicine patient care experience. The Family Medicine Nurse Practitioner practices in a team-based care model and collaborates with other members of the team to care for a panel and a population of patients. The Family Medicine Nurse Practitioner refers patients for medical care management and care coordination services with internal staff and/or with other agencies. The Family Medicine Nurse Practitioner is responsible for timely follow up of specialty referrals and diagnostic testing. The Family Practice Nurse Practitioner prioritizes meeting and exceeds quality care measures as identified by GBMS. 

 

Full-time hours for this position include thirty-two (32) clinical and eight (8) administrative hours with no weekends currently. Subject to change. 

Essential Functions: 

  1. Provides comprehensive physical assessment of adult patients and evaluates the outcome of patient care. 

  1. Prescribes or recommends forms of treatment such as pharmaceutical (drug) therapy, physical therapy, inhalation therapy, or related therapeutic procedures. 

  1. Conducts age appropriate risk assessments and developmental assessments  

  1. Assures quality of patient care by participating in the organization’s Performance and Quality Improvement activities. 

  1. Advocates and orders immunizations according to current ACIP and/or CDC guidelines. 

  1. Arranges referrals, consultations, therapeutic services, and confers with other specialist on course of care and treatment. 

  1. Establish medical diagnosis for common short-term and chronic stable health problems.  

  1. Order, perform, and interpret laboratory and diagnostic tests. 

  1. Order and perform diagnostic, therapeutic, and corrective measures. Prescribe drugs 

  1. Provide same-day care as required. 

  1. Refer patients to appropriate licensed physicians or other health care providers as needed.  

  1. May refer patients to physician for consultation or to specialized health resources for treatment. 

  1. Provides direct patient care as needed and appropriate to serve the community in accordance with the organization’s mission, vision, and values 

  1. Provides appropriate documentation of clinical care provided in a timely manner. This includes maintenance of patient charts in accordance with clinic protocols, and timely documentation of clinical procedures on encounter forms. 

  1. Complies with regulations regarding consultations and other services as required by patients’ insurance coverage. 

  1. Complies with Joint Commission standards for ambulatory care. 

  1. Complies with Patient Centered Medical Home standards and guidelines 

  1. Performs timely entry into, and manipulation of, the Electronic Health Record.  

  1. Completes chart documentation timely in accordance with the company policy. 

  1. Complies with the organizations training requirements and risk management plan/programs. 


Nonessential Functions: 

  1. Provides clinical leadership and acts as a resource to other members of the health care team, patients, or other members of the community. 

  1. Provides clinical support and acts as a resource to the Chief Medical Officer and other providers working in the clinic as appropriate. 

  1. Works with center manager to facilitate smooth operation of clinic flow. 

  1. Cooperates with compliance review activities. 

  1. Submits information needed for credentialing files in a timely manner. 

  1. Participates in team/staff meetings; attends in-service meetings and applicable workshops.  

  1. Performs any and all other duties required to provide for optimal patient care. 

 

Supervisory Responsibility 

  1. CMAs, fellows, students, interns and the like if applicable 

 

Minimum Qualifications 

  1. Current License to practice medicine in the State of Maryland. 

  1. Current Maryland prescriber number, and current D.E.A. and C.D.S. License. 

  1. Nurse Practitioner Board certification preferred. 

  1. Current Cardiopulmonary Resuscitation Certificate


Work Environment 

This job operates in an outpatient office environment.

Travel 

Must be able to drive between Greater Baden Medical Services, Inc. locations.
