We are seeking an Integrated Community Health Worker (ICHW) who not only has a passion for connecting with patients to meet their self-identified goals/needs and address barriers to care (transportation, food insecurity, health care/resource/benefits navigation, housing, language access, etc.), but is equally interested in working within a cohesive, highly collaborative team based in a clinic environment. ICHW are available to respond in real time to identified patient needs both in-person and through phone outreach. Within this fast-paced environment, they also must balance availability for outreach with a variety of administrative tasks within the workday, such as data collection, resource research, and more.

The ideal candidate is a people-person who is outgoing, with the ability to remain patient-centered and trauma-informed in their approach with patients and the care team. We are looking for clear and concise communicators, quickly able to establish rapport and provide information to patients within a short amount of time in the exam rooms. Good boundaries and time management skills are highly desirable.
