Sage Solutions provides personalized, holistic, care management for older adults in Connecticut. We are seeking an experienced Care Navigator to join our existing team in Hartford. Care Navigators assess the needs of older adults and provide thoughtful care planning, advice, and strategies. We advocate for our clients' and collaborate with stakeholders to maximize wellness. Ideal candidates will have a passion for helping others and strive to uphold our mission to improve the quality of life of all clients.
This is part-time, per diem work. Weekday, daytime availability is required. Hours fluctuate and can largely be set by the Care Navigator.
Requirements
REQUIRED QUALIFICATION & EXPERIENCEEducation:
Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, nursing, or related field required.
Skills:
Patient advocacy
The ability to work within family situations
The ability to communicate with professionals, clients, and stakeholders in a clear, professional, and caring manner
Strong written, verbal and computer skills required
Ability to maintain confidentiality of client, company, and staff information
Experience:
Must have a minimum of 10 years of experience in health care or social services or related field plus experience working with older adults. Applicants without 10 years of experience working with older adults will not be considered for the position.
Knowledge and understanding of social, health, and economic factors that contribute to health and wellbeing outcomes; knowledge and skill in interviewing and assessment (social and health) techniques.
Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care.
Other:
Reliable transportation, valid driver’s license, and current automobile insurance.
The hourly rate is commensurate with experience.
