POSITION SUMMARY: Responsible for the organization and planning of Social Services for current and prospective
residents. Provides services that support residents in maintaining or achieving their highest practicable physical,
mental, and psychosocial well-being. Actively participates in and coordinates the preadmission, admission,
discharge, and post-discharge follow-up process and coordinates care through collaboration with interdisciplinary
teams. The role includes administrative oversight of admissions and discharges, ensuring regulatory compliance
and a smooth transition for residents and families. Requires collaboration to ensure full occupancy by reviewing
referrals and admissions.
ESSENTIAL FUNCTIONS:
• Represents the campus in a positive manner while creating an environment that fosters respect and courtesy
for residents and employees alike. Works within the campus programs to promote safety and the well-being
of all residents and employees.
• Upholds the campus’s Mission Statement.
• Maintains HIPAA standards and regards all medical or healthcare information pertaining to residents and
employees as confidential.
• Ensures that the administrative and clerical functions related to the admission and/or discharge of residents
is carried out to completion by coordinating efforts with the appropriate Team Members, Residents, and
Family Members. Assists with the responsibilities and functions related to discharge planning including setting
care plan with family members, ensuring DME as ordered and coordinating discharge plan with
Interdisciplinary Team.
• Maintains consistent and appropriate contact and relationships with local hospitals, doctors and outside
agencies to facilitate admissions.
• Coordinates the forms needed for admissions and enters admission details and notes into electronic
recordkeeping systems accurately and promptly and in accordance with Company standards.
• Maintains state required resident roster, coordinates financial updates, and completes appropriate section of
the MDS.
• Coordinates with the Home Office Regional Admissions Coordinator(s) daily ensuring communication with the
community’s nursing team.
• Maintains the admissions process according to Company standard, including admission packet completion
and resident guides before a resident’s admission.
• Manages and processes timely and accurate insurance verification and updates for skilled nursing stays,
including Medicare Part A and Medicare Advantage (if needed), Medicare Part B, and private insurance.
Requires inputting verification details, and the HIPPs code (as applicable), in the resident’s chart promptly
when a new verification is received.
EDUCATION AND EXPERIENCE REQUIREMENTS:
• Bachelor’s degree in Social Work, or closely related human services field required
• 1-2 years related experience in a Healthcare setting.
• Two years’ experience in a similar social work position required.
• Two years of long-term care or rehabilitation experience desired.
