Location: Merrimack, NH
Shifts: 7pm to 7am or 11pm to 7am for 32 hrs. to 40 hrs. a week; sharing in every other weekend and agency holiday rotation-one summer and one winter holiday

Responsibilities:
- Complete an initial assessment of patient and family to determine hospice care needs. Provide a complete physical assessment and history of current and previous illness(es). Utilize the nursing process in planning, providing, and evaluating skilled needs.
- Accessing, collecting data, and evaluating the patient's physical, spiritual, and emotional needs.
- Provides preventative and therapeutic treatment under medical direction.
- Creates a nursing plan of care with input from the patient, family, and provider to include measurable goals and behavioral objectives.
Qualifications:
- Graduate of an accredited school of nursing.
- Associate's degree in nursing.
- Strong skills in symptom management.
- One year recent acute care experience.
- Experience managing oncology patients and those with co-morbidities.
Preferred Qualifications
- Bachelor’s degree in nursing.
- Community Health/Education experience.
- Certification in hospice and palliative care a plus.
- Knowledge or experience with end-of-life care and hospice care is beneficial.
- EMR software proficiency. We use EPIC.
Required
- Registered as a RN with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Position requires sharing in every other weekend rotation and agency holidays.
