Responsibilities / Essential Functions:
Core Clinical Competencies:
Mental Health Social Workers are required to complete thorough assessments of clients, which includes sufficient documentation of symptomatology and functioning levels to support the diagnosis, the global level of functioning score indicated, and medical necessity.
Develops quality treatment plans as indicated by the inclusion of the assessment presenting problem as the primary focus of the treatment plan, with treatment designed to reduce, eliminate or maintain where appropriate the primary behaviors, emotions, cognitions associated with the presenting problem.
Organizes time and caseload such that all required paperwork and treatment plan reviews are completed within the established time frames.
Assesses and includes accurate GAF scores within assessment and discharge documentation.
Performs discharges of clients, which meet established regulatory standards and requirements.
Responsibilities / Essential Functions:
Mental Health Social Workers are required to complete quality, accurate and timely service notes based on medically necessary services that have been rendered to the clients and meets agency productivity standards.
Oversees the condition of assigned agency client files to ensure that documentation required is completed and up to date.
Provides on-going problem solving, remedial counseling or individual therapeutic counseling to clients on an as needed basis or as clients’ treatment plan dictates.
Completes quality checks on staff progress notes as outlined in agency policy and procedure.
Oversees the operation, staff and client care of assigned residential sites.
Provides annual Performance Evaluations, monthly team meetings and individual weekly supervision to Program Managers to provide guidance, support and address problem behaviors.
Leads the clinical team in support of the client’s treatment plan.
Coordinates with staff on site, psychiatrist, physician, family and other providers to ensure continuity of care, and transitional planning during admissions, discharge and transfers to other programs, hospitals, etc.
Provides for clinical, crisis and administrative on-call coverage for agency as assigned. On-call coverage includes after business hours, weekends, and holidays.
Responsible for 24 hour scheduling coverage of assigned sites. Obtains coverage for vacation / call offs.
Participates in staff orientation, training and development. Attends all mandatory training, and seeks out additional training outside agency to enhance professional development, maintain current license and meet continuing education requirements.
Adheres to clients’ rights under the Mental Health Code, Confidentiality Act, HIPAA and Rule 50 of the Office of Inspector General, abides by Stepping Stones Code of Ethics and Professional Practice.
Adheres to Stepping Stones’ polices and procedures.
Performs other duties as assigned by CEO and DS.
Contacts:
The Mental Health Social Worker will have contact with the CEO, DS, BGD, other Mental Health Social Workers, Program Managers, Mental Health Professionals, Rehabilitation Aides, Agency Psychiatric Medical Director, physicians, administration and support staff, quality assurance and computer staff, maintenance staff, clients, families, guardians, numerous outside agencies and service providers.
Qualification Requirements:
To perform this job successfully a person must be people oriented, motivated, flexible, able to handle multiple responsibilities, and organized. Requires skill in conflict resolution, problem solving, and prioritization. The ability to lead individual, separately and collectively and exercise good judgment and problem solving skills is also required. Must have a valid driver’s license, liability insurance and own and operate passenger vehicle for work.
Supervisory Responsibilities:
The Mental Health Social Worker is responsible for the overall operations of the sites and the general well being of all clients assigned to them. The Mental Health Social Worker supervises Program Managers, Mental Health Professional, Rehabilitations Services Aides, and other staff as assigned.
Education:
Minimum formal requirement: Either a Masters degree in human services, or an RN license. Individuals should have 5 years minimum experience in human services and one year of administrative or supervisory experience is preferred. This position meets the qualifications of Medicaid Rule 132, Qualified Mental Health Professional.
Physical Requirements:

While performing the duties of this job, the employee will frequently be required to sit and use a keyboard (approx. 4 hrs.), walk, reach and bend (approx. 4 hrs.), and lift (30 lbs.), and drive an agency van or passenger car. Employee must be able to communicate well in both oral and written format.
Work Conditions:
Environment is moderately noisy with most activities taking place indoors. The Mental Health Social Worker works an 8am to 5pm day and a 40-hour workweek, with some on-call responsibilities. There is risk involved with client contact. While working direct support persons must not be concurrently engaged in child or family care, personal business or errands, personal hobbies, or any other pursuit which would diminish their effectiveness in the provision of necessary and appropriate programming, including ensuring the safety and well-being of those in their care.
