Position Summary:
Project HOPE (Hepatitis C and Overdose Prevention Empowerment) is a program which provides low-barrier and accessible hepatitis C treatment and navigation in high impact settings such as syringe exchange sites, opioid treatment sites, and various drop-in services as well as sexual health clinics which aim to reach target priority populations. This program utilizes incentivization for picking up stored medication in a timely manner. Project HOPE Program Specialist will work across various SFCHC sites including the Community Living Room at 730 Polk, Trans Thrive at 1460 Pine, and Taimon Booton Navigation Center to deliver HCV, HIV, and overdose education and treatment services to community members. The Program Specialist will also assist the Community Engagement team with its various projects and programming including the Community Living Room, and will report to the Program Manager of Community Outreach & Linkage.
Essential Duties:
- Work collaboratively with direct supervisor and clinical team on to ensure that program activities are implemented and completed successfully;
- Conduct city-wide outreach to engage and assess individuals at risk for Hep C, HIV, and accidental overdose and provide health education and linkage support to supportive programming within SFCHC and throughout San Francisco.
- Develop and facilitate support groups for clients to address risks for HIV, Hep C, and accidental overdose, and monitor client progress throughout experience.
- Coordinate with SFCHC Wellness Clinic providers to support clients living with Hep C with linkage and follow-up support, providing outreach and navigation assistance, as needed, to support with treatment adherence and engagement.
- Become certified as a HIV and Hep C Test Counselor through SFDPH and provide as-needed rapid testing and counseling.
- Work collaboratively with direct supervisor, Community Engagement department management and staff, and community partners to ensure high quality service delivery for agency guests, clients, and patients.
- Provide high quality customer service to guests, clients, and patients accessing SFCHC programming and care.
- Support guests, clients, and patients, with linkage and navigation support to other SFCHC programming as requested.
- Coordinate supportive programming for the Community Living Room and SFCHC’s Wellness Clinic; work with the Community Engagement team to maintain a regular calendar of activities for community members, facilitate or coordinate other staff and guest speakers for educational presentations and workshops, and ensure that space is available/reserved for activities.
- Work with the Program Manager for the Community Living Room to make sure supplies and materials needed for activities and events are ordered ahead of time and available for use.
- Create and distribute media including, but not limited to, promotional flyers for events and activities, educational presentations for the Community Engagement department as well as for the Wellness Clinic, and educational pamphlets and materials for distribution.
- Act as an ambassador of the organization and represent the work of the agency to the community including at street fairs, at partnering organizations, and at community events.
- Complete timely documentation of assignments and any other required documentation including, but not limited to, client interaction notes, trainings, responses to emails, presentations, etc.
- Function in a supportive role for the various programs and projects of the Community Engagement team.
- Participate in case conferences, staff meetings, in-service trainings, and supervisions.
- Perform other duties and tasks as assigned.
Minimum Qualifications:
- Ability to sit, stand, and walk frequently (this position requires community outreach)
- At least one year of experience providing counseling, advocacy, education and service coordination and/or outreach work to disenfranchised communities, including people living with Hep C, HIV/AIDS, transgender and gender non-conforming identified individuals, and people experiencing homelessness and housing instability.
- Knowledge of HIV/AIDS, STIs, substance use, mental health, and social services.
- Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques.
- Must be adaptive, flexible, and a team player.
- Strong verbal and written proficiency in English.
- Ability to work weekends and evenings occasionally.
- Bilingual language skills preferred.
- Ability to maintain HIPAA compliance, confidentiality, and privacy of people, documents, data, and communications.
- Up-to-date with vaccinations, especially the latest COVID-19 and flu vaccines.
- Proof of TB clearance (issued within the past 12 months).
