Description

The Senior Case Manager is responsible for the coordination and delivery of quality direct case management services for Chamblet and Revision Family Home. The Senior Case Manager will provide consultation to interdisciplinary team members on issues related to supporting residents’ housing, development and facilitation of treatment plans with residents, and assisting the Program Director with weekly case review. The Senior Case Manager reports to the Program Director for weekly supervision and programmatic assignments.


Location: Chamblet Family Home in Dorchester provides life-stabilization services, intensive housing placement services and vocational case management services to homeless families and to pregnant women. Our program helps families increase self-sufficiency, gain healthy nutritional knowledge, search for more permanent housing and build job-readiness skills.


Essential Functions:

  • Act in accordance with Victory Programs, Inc.’s Standards of Excellence & Philosophy of Care at all times including maintaining professional ethics, boundaries and client confidentiality.
  • Provide weekly collaborative case management services to 14 families to work collaboratively to assist the residents in addressing any barriers they are facing surrounding housing, finances, vocation and/or education, physical and mental health, childcare, and legal issues.
  • Assess and provide referrals to external providers as needed.
  • Develop and maintain service linkages and databases to support residents including developing relationships with local service providers and housing providers.
  • Accompany clients to housing, legal, and medical appointments as needed.
  • Develop and monitor service plans as required by EOHLC as well as complete timely reporting of housing assessments, case notes, and relevant data into the HMIS/ETO system and the residents’ case files.
  • Develop, implement, and facilitate workshops, community meetings, and resident events in conjunction with the Shelter Manager and Program Director as required by the funder and agency.
  • Work closely with all Case Managers to provide resources and support with developing and implementing service plans for residents and stabilization families.
  • Facilitate weekly case review and complete required documentation.
  • Complete weekly HMIS/ETO audits and implement plans for timely completion of assessments in conjunction with the Program Director.
  • Assist with program coverage as needed by maintaining building security, adhering to all program policies and protocols of the residences, and completing all required documentation in the program shift logs and staff communication logs.

Non-Essential Functions:

  • Attend supervision, staff meetings, clinical meetings, in-service training, and agency workshops.
  • Participate and support the agency in program events, State House rallies, fundraising and advocacy events, rallies, walks and any additional functions.
  • Transport clients using agency vehicles.
  • Perform any other duty or task that may be requested from time to time by your supervisor or their designee.

Qualifications: 

  • Master’s Degree in Social Work, LADCI, or in another human services field preferred.
  • Minimum of three years’ experience in housing and client services is required.
  • Knowledge of family shelter services or residential services and understanding of harm reduction philosophy required.
  • Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors preferred. 
  • Openness to harm reduction philosophy.
  • A valid Massachusetts driver’s license with a safety driving points of 00, 99, 01, 02, or 03. 
  • Ability to work independently and cooperatively as a member of a professional team.
  • Strong organizational, written, and verbal communication skills.
  • Strong computer skills, including Google office suites and Internet use.
  • Demonstrated ability to work successfully in multicultural settings with diverse client groups, including people with mental health concerns and substance use disorder.
  • Must be CPR certified (may be obtained upon hire)
  • Complete TB screening (may be obtained upon hire)
  • Three strong work-related professional references; at least 2 from manager or supervisor.

Preferred Qualifications:

  • Bilingual (English/Spanish, Haitian Creole) preferred.

Position Type: Tuesday-Saturday (Tuesday 11am-7pm, Wednesday through Friday 8am-4pm, Saturday 11am-7pm) On site.

Rate: $26.00/hr
