JOB SUMMARY
While conforming to state regulations and Gellert Health policies and procedures, the Supervisor of Navigation (S o N), under the leadership of the Director of Navigation, will focus on the following priorities:

JOB RESPONSIBILITIES
1. TEAM MANAGEMENT AND LEADERSHIP
The S o N provides leadership, supervision, and weekly guidance to Navigators. Within the context of cultural competence and trauma informed care. The S o N will:
· Supervise, train, and mentor a team of Navigators.
· Monitor case progress and ensure that patients' needs and goals are being met.
· Provide guidance, support, and direction to ensure the team's performance meets organizational goals.
· Lead team meetings, provide feedback, and address any performance or interpersonal issues.
2. NAVIGATION OVERSSIGHT
· Ensure S o N has a thorough working knowledge of their supervisee’s patients.
· Review and assign cases to Navigators, considering workload and skill level.
· Ensure compliance with organizational policies, procedures, and regulatory requirements.
· Find coverage for supervisees as needed.
· Be a point of contact to Supervisees with respect to administrative and practical requirements of the job.
· Problem-solve with Navigators regarding:
o Challenging Cases
o Social Determinants of Health
o Lack of resources
o Maintaining boundaries
o Encouraging self-care
o Complying with GH policies and procedures
3. QUALITY ASSURANCE
· Implement and maintain quality assurance standards for Navigation services.
· Conduct regular audits and reviews of files to ensure accuracy and adherence to established protocols.
· Provide close monitoring and reviews of:
o Electronic Medical Record
o Compliance
o Administrative requirements
4. PATIENT ENGAGEMENT
· Handle escalated patient issues and concerns that require supervisor intervention.
· Collaborate with Navigators to develop and implement effective plans.
· Ensure that patients receive appropriate and timely services based on their individual
needs.
5. INTERDISCIPLINARY COLLABORATION
· Represent Gellert Health at meetings with referral sources with respect to patient status and progress.
· Collaborate with other organizations and community partners to coordinate services and resources for patients.
· Facilitate communication and information sharing among various stakeholders involved in a patient's care.
6. STAFF DEVELOPMENT
· Identify training needs and opportunities for professional development among
Navigation staff.
· Plan and deliver training sessions to enhance the skills and knowledge of the team.
· Provide training to Navigation Staff during Orientation
· Assist staff with developing skills to carry out Navigation duties.
7. PROGRAM DEVELOPMENT
· Contribute to the development and enhancement of Navigation programs,
policies, and procedures.
· Recommend and implement process improvements to increase efficiency and
effectiveness.
8. CONTINUOUS QUALITY IMPROVEMENT
· Stay informed about industry trends, best practices, and new developments in Navigation.
· Continuously assess and adjust the Navigation approach to align with
evolving patient needs and organizational goals.
· Maintain accurate and up-to-date patient records and case documentation.
· Generate reports and analytics to track case outcomes, trends, and service
effectiveness.
· Use data to make informed decisions and improve the overall quality Navigation services.
All other duties as assigned by the Director of Navigation.

QUALIFICATIONS & REQUIREMENTS:
· At least 21 years old
· External Applicants: Experience as a Supervisor in Case Management or a
related field
· 39 months clear Motor Vehicle Record
· Auto insurance.
· Ability to successfully pass a criminal background check.
· Associate degree or bachelor’s degree, or work experience equivalent.
· Experience working with culturally diverse, chronically homeless, and vulnerable individuals, in medical and behavioral health services (preferred).
· Clean and working four-door car
· Current Arizona driver’s license
· Bilingual – Spanish highly desirable not required.

PROBATIONARY PERIOD
The first 90 days of new employment or when accepting a new position shall be a probationary period and your employment may be terminated during this period at any time without notice during your first month, and with one week’s prior notice thereafter. A Gellert Health employee who accepts a new position in the company and fails at securing the new position may be offered a return to their previous position when available.
GH may, at our discretion, extend this period for a further 90 days. During this probationary period your performance and suitability for continued employment will be monitored.
New hires receive the following during the probationary period:
· Extensive training and support from the Director of Navigation
· Weekly check-ins to receive feedback
· Support for improving performance to meet our standards and expectations
· A written performance evaluation at the end of the probationary period
Upon successful completion of the probationary period, the employee is granted permanent employee status. Completing this probationary period does not guarantee continued employment with Gellert Health. Employment is at-will and can be terminated at any time with or without cause. The employee is subject to all company policies and procedures.
