Our programs
BadgerCare Plus: CCHP, the second largest BadgerCare health plan in Wisconsin serves over 130,000 adults and children in 27 counties in eastern Wisconsin.
Care4Kids: We're the pioneers of Wisconsin's Foster Care Medical Home initiative, providing vital health care coordination services to 2,800 children in out-of-home care in Southeastern Wisconsin. Through our "medical home" model, we strengthen care quality, access, and timeliness, shaping healthier futures for these children.
Individual and Family Plans: Tailored to meet the diverse needs of individuals and families in a 15-county service area, our plans serve more than 13,000 members. Hybrid Role (Primarily Remote)
1st shift - 8:00am-4:30pm (no routine weekend or holiday commitment)
Supports CHORUS Community Health Plans (CCHP) members with chronic health conditions utilizing an integrated, comprehensive approach to care. Assists in the direct and indirect coordination of care through member, caregiver, practitioner and community education while addressing medical, behavioral, educational and social needs. Mostly telephonic outreach and communication.
Bachelor's degree in Nursing (BSN) required within 1 year of hire
Registered nurse with a valid, unrestricted Wisconsin nursing license.
Two or more years of nursing experience. Experience in a Medicaid HMO/managed care or public health setting preferred.
Strong organizational skills.
Demonstrated ability to prioritize tasks and handle multiple issues while meeting timelines.
Excellent oral and written communication skills.
Ability to communicate effectively and work collaboratively with internal departments, external providers and members.
Demonstrates teamwork within the department and community by assisting other staff members and/or organizations as necessary.
Independent judgment, analytical ability, systems thinking and interpersonal communications to work in complex systems.
Awareness of community organizations and resources to address member and family needs.
Computer knowledge and skills in word processing required.
