Home Connections Mission is to help ease patients through the course of serious illness, regardless of changes in diagnosis, medical treatment or location.

Home Connections (Palliative Care) is an affiliate of Hospice & Palliative Care Buffalo, which serves patients through home visits and works in partnership with the patient's physicians to coordinate care and help navigate the complexities of today's healthcare system.

A Register Nurse (RN) Manager for Home Connections is responsible for the daily supervision of palliative care services provided to patients in the Home Connections community-based adult palliative care program. Manages and coordinates with the interdisciplinary team and is responsible for the clinical oversight and overall supervision of staff on the interdisciplinary team. In collaboration with the VP of Palliative Care, assists with program development and outreach strategies.

Essential Duties and Responsibilities for a Register Nurse (RN) Manager for Home Connections:

Provides oversight and direction for the clinical staff and office support staff in the delivery of palliative care services.

Coordinates palliative care interventions in collaboration with the interdisciplinary team and the patient's primary physician and medical team consistent with program goals, verifiable through outcome measures such as Patient Satisfaction Survey, Utilization Measures, and Clinical Outcomes.

Handles recruiting, hiring, evaluating, and terminating personnel and making staffing pattern recommendations to assure adequate, appropriate, and clinically competent staffing levels are maintained.

Prepares staffing schedules as appropriate and anticipates staffing profile needs proactively. Assures 24/7 nurse on-call availability for both phone and direct patient contact.

Collaborates with the Director of Palliative Care, CCOO, and Director of Quality Improvement in the implementation of Home Connections program performance improvement projects to improve program services and patient/physician satisfaction with services.

Uses leadership position to set positive, attainable expectations, objectives, and goals for staff development.

Provides guidance and support to staff, in the planning of care management services to ensure that services provided meet the needs of diverse patient populations and problems that arise, including appropriate risk management interventions.

Facilitates and documents a collaborative, efficient and effective IDG Team meeting that is focused on both patient-family goals of care and optimal management of the palliative needs of the patient and family.

Participates in case conference meetings with payers and other community partners in care.

Assures timely documentation of all patient visits and contacts including timely updates to the primary physician through sharing of visits notes and appropriate documentation.

Meets quality improvement standards relative to chart audits to assure proper documentation of services provided.

Adheres to HIPAA regulations and policies. Assures the security of patient electronic/written records.

Elicits no founded patient/family complaints or any preventable or unusual occurrences.

Adheres to requirements of HPCB policies and procedures that are both patient focused (clinical) and organizational focused (administrative).

Maintains an appropriate schedule of supervisory field visits and assures that staff adheres to yearly Department of Health (DOH) mandatory requirements.

Plans and participates in the orientation, mentorship, and staff development needs of personnel to assure that program personnel are current in clinical information and practices.

Shares in the manager on call rotation and responds to clinical on-call needs with appropriate interventions and staff guidance.

Occasionally called on to perform patient visits, independently or jointly with other IDG members.

Qualifications for a Register Nurse (RN) Manager for Home Connections:

Education Certification

Graduate of an accredited school of nursing with current RN licensure in the State of New York.

BSN preferred.

Certification in Hospice and Palliative Nursing (CHPN) encouraged within 1-2 years of employment.

Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call.

Experience

Minimum of 3 years of clinical nursing experience required.

Hospice and/or palliative care experience preferred.

Experience in homecare or community-based services helpful.
