What You Will Do
- Participates in development, implementation and evaluation of NFP policies, procedures and other elements of the NFP Program to ensure achievement of goals and compliance with NFP, VNS Health and regulatory standards.
- Uses principles of supervision, reflective practice, and staff development, to provide primary support and appropriate oversight to the nurse home visitors, and to build and model a strengths-based, client-centered culture.
- Monitors compliance with NFP, DOH and VNS Health goals and objectives, and develops team strategies for improvement.
- Collaborates with the DOHMH and agency programs and departments to design and implement marketing and customer relation initiatives. Promotes NFP Program through liaison, lecturing and promotional activities in the community.
- Manages service delivery of assigned NFP team; ensures achievement of team goals and performance measures within the NFP and Agency’s strategic objectives and assumes accountability for achievement of team’s clinical, process and fiscal performance outcomes.
- Observes and analyzes performance patterns and uses reflective supervision to develop interventions to improve individual and team performance. Provides guidance and support to assigned team to encourage and support team learning, implementation and evaluation of plans that maintain and increase customer response, cost-efficiency, and quality care compliant with NFP and regulatory standards.
- Ensures that all staff, including supervisor, attend all required NFP Initial Education sessions as outlined in the orientation materials and contract with the Nurse-Family Partnership National Service Office.
- Acts as a key resource and/or liaison in providing clinical and operational guidance and support to team. Uses NFP, DOHMH and VNS Health resources to achieve and enhance team outcomes. Supports, collaborates and provides guidance to NFP nurses in working through team development issues.
- Develops staff through individual and team coaching, use of internal and external resources and participation in NFP education and training sessions to build confidence and skill of nurse home visits. Involves staff in the process of self-development and holds staff accountable for meeting performance expectations and development plans. Uses a variety of methods in obtaining information about team member’s performance (e.g., team meetings, home visits, frequent chart review, and feedback from other professionals and clients).
- Analyzes and helps to resolve complex clinical case management problems, working in collaboration with other NFP managers, clinical specialists, and other VNS Health professional staff, as appropriate. Provides guidance to individuals and team to identify and resolve internal and external customer issues.
- In consultation with team members, identifies staffing and service needs of designated geographic areas and recommends changes as needed.
- Interprets and implements NFP/DOHMH and agency policies, state and federal regulations, and collective bargaining agreements.
- Collaborates with other NFP team members to develop and maintain relationships with community health and social service providers to ensure reciprocal referral relationships that support NFP enrollment requirements and access to resources that meet family needs.
- Performs all duties inherent in a managerial role. Ensures effective staffing, evaluates performance, provides input for the development of the NFP budget, and hires, promotes, and terminates staff and recommends salary actions, as appropriate.
- Participates in special projects and performs other duties as assigned.
Licenses and Certifications:
- License and current registration to practice as a Registered Professional Nurse in New York State Required
Education:
- Bachelor's Degree in Nursing Required
- Master's Degree or equivalent experience Required
Work Experience:
- Minimum of four years experience in a health care or human services program, inclusive of a minimum of two years of managerial experience, a minimum of one year working with new mothers and infants and a minimum of one year working in a community based or home visiting program Required
- Proficiency in personal computer operations including MS Office applications Required
