Trio Hospice / Jet Health Inc. is currently seeking an experienced Hospice Team Coordinator to provide administrative and clerical support to hospice team members, patients, and families enrolled in our hospice services for our Denton, TX hospice office.
This role is responsible for maintaining accurate medical records, ensuring required physician signatures and documentation are obtained, coordinating team schedules, and facilitating communication across staff, patients, families, and external partners. The Team Coordinator plays a vital role in supporting efficient operations and quality patient care.
Requirements
- Maintain patient medical records in compliance with regulatory requirements.
- Obtain required physician, staff, and other signatures for authorizations, certifications, and physician orders.
- Screen calls and facilitate communication between managers, staff, patients, families, and external partners.
- Manage daily schedules for team members; provide patient updates to clinical staff at end of day.
- Enter patient care information into electronic systems; monitor event tracking (e.g., recertification dates); support Billing with documentation as requested.
- Prepare agendas, distribute materials, and attend weekly team meetings.
- Order and track DME, ensuring timely documentation of orders and deliveries.
- Draft correspondence and provide information to support staff, patients, and families regarding hospice care and eligibility.
- Provide backup support for Receptionist as needed.
- Research and communicate vendor or facility resources; coordinate follow-up with outside agencies when required.
- Promote and support a collaborative, professional, and positive work environment.
- Adhere to safety standards and maintain a productive, organized workspace.
- Perform other duties as assigned.
Qualifications:
- Minimum of 3 years of hospice or healthcare experience preferred.
- Familiarity with medical terminology required.
- Strong communication, phone, typing, and computer skills
- Ability to multitask, prioritize, and manage time effectively.
- High attention to detail and accuracy in documentation.
- Professional, positive, and compassionate demeanor with patients, families, and staff.
Physical & Work Environment Requirements:
- Frequent sitting; occasional standing, walking, stooping, or reaching.
- May occasionally lift to 10 lbs.
- Ability to work at a computer, laptop, or mobile device for extended periods.
- May require travel to other locations.
- Ability to adapt to changing responsibilities and manage occasional stress.
- Moderate noise level in office environment.
