- Office in Grand Rapids
About the Department
Nurse Family Partnership works by having specially trained nurses regularly visit first-time moms-to-be, starting early in the pregnancy, continuing through the child’s second birthday. Mothers, babies, families and communities all benefit.
This evidence-based model of partnering nurses and first-time moms has more than 40 years of research from randomized, controlled trials that prove it works. The program is designed to improve pregnancy outcomes, improve child health and development, and improve the economic self-sufficiency of the families of low-income, first-time mothers and their babies. Services begin during pregnancy and continue until the child is two years old. Duties will also include coordinating the delivery of physical and behavioral health, social, economic, and/or other services to clients.
"Being a home visiting nurse with Nurse Family Partnership allows me to walk alongside and support women as they prepare for and navigate motherhood. There is nothing more fulfilling and empowering than watching a client achieve their goals." -Sophie Gwasdacus, Public Health Nurse
"I am grateful for the opportunity to connect with first time moms and witness positive changes due to the nurse/client relationship. It is a rewarding position and I have grown in my skills as a nurse by learning the NFP model." -Becky Doucette, Public Health Nurse
