About the Role:
The Physical Therapist PRN at Woodhaven Village in Conroe plays a critical role in delivering high-quality rehabilitative care to residents within the assisted living facility. This position focuses on assessing, planning, and implementing individualized physical therapy treatment plans to improve patients' mobility, reduce pain, and enhance overall functional independence. The therapist collaborates closely with interdisciplinary teams, including nursing staff and physicians, to ensure comprehensive care and optimal patient outcomes. Flexibility and adaptability are essential, as this PRN role requires providing therapy services on an as-needed basis, responding promptly to patient needs and facility demands. Ultimately, the role aims to support residents in achieving their highest possible level of physical function and quality of life within the facility setting.
As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
