Your role at BHSN:
As a Program Supervisor for the Breakthrough II Community Residence at BHSN, you will help lead and support the community residence team and the residents of the Breakthrough II.
Work Schedule: Monday-Friday 8am-4pm or 8:30am-4:30pm

What your day might look like:
- Facilitates, develops and assists with mentoring, on-boarding, training, supervising and monitoring community residence Support Specialists Levels I, II and interns;
- Establishes and coordinates staff schedule to ensure adequate coverage is maintained to ensure adequate coverage 24/7, 365 days/year;
- Collaborate and communicate effectively with team members to ensure resident needs are met including but not limited to: coordination of mental health/substance use/primary care services, obtaining funding, transportation, linkage to other services, care coordination and discharge planning
- Coordinates and monitors that monthly site safety checks and resident fire drills are completed correctly and any deficiencies resolved promptly to ensure a safe and clean living environment
- Promptly reports facility/maintenance/safety concerns to Program Coordinator and Facilities Director to ensure a safe living environment for the residents
- Collects and organizes various data throughout the year as requested (i.e. client satisfaction surveys, occupancy rates, annual reports, etc)
- Coordinate, facilitate and ensure accurate documentation of resident house meetings at least monthly
- Coordinate and participates in regularly scheduled treatment team meetings and effectively communicates outcomes to team members to ensure resident’s needs are met
- Provides, monitors and ensures that all participants receive restorative services as indicated on their service plan and accurate documentation of services in a timely manner in accordance with Federal/State regulations and BHSN policies/procedures
- Complete NYSCRI Adult Residential Assessment with participant and assist participant to develop an individualized service plan outlining participant’s goals/objectives and what restorative services will be provided to assist the participant
- Develops service plans following the eleven restorative services as required by Office of Mental Health regulations and documents progress
- Observe the resident self-administration of medication according to established protocols
- Works with Adult Residential Nursing Team to maintain accurate medication records, storage, handling and monitoring of medications
- Assist residents in maintaining safe and sanitary dwelling
- Complies with all BHSN and regulatory policies and procedures
- Documents all services in accordance with Federal/State regulations and agency policies/procedures
- Collaborates, documents and timely communicates client incidents/events to appropriate BHSN program, i.e. PROS, Care Management, clinics
- Ensures adequate food and supplies are on hand at all times to meet resident / program needs
- Facilitates, develops, ensures documentation of and participates in resident activities/outings
- Additional duties as assigned within scope of Job Description
Your skills and qualifications:
- Bachelor’s degree in human services or medical field strongly preferred
- Management experience preferred
- 3 years’ experience with vulnerable population required
- Experience with crisis resolution preferred
- Strong interpersonal, communication, time management, problem solving, leadership, and organizational skills required
- Reliable transportation to transition between varying program sites required
- Must be able to lift up to 25 lbs. and have the ability to climb and descend stairs
- Ability to work a flexible schedule to accommodate the needs of the program
- Valid NYS driver’s license, acceptable to BHSN insurance carrier, for required local travel
- CPR and NARCAN certification required within 60 days of employment
- Includes work hours during non-traditional business hours
