Position Summary:
Under the general direction of the Director of Case Management and in collaboration with physicians and hospital staff, evaluates and implements treatment regime for designated psychiatric and substance abuse patient population, assists with reporting suspected abuse for all age groups, assists uninsured with eligibility process, assists case managers with hospital follow up post discharge to prevent hospital readmission.
1. Education/Qualifications:
Master’s degree from an institution accredited by the Council on Social Work Education
A Home Health Master of Social Work (MSW) provides psychosocial support, counseling, and resource navigation to patients and their families in their homes, addressing physical, emotional, social, and financial challenges related to illness, injury, or recovery. Key responsibilities include assessing needs, developing care plans, collaborating with healthcare teams, advocating for patients, maintaining documentation, and connecting them with community resources to promote independence and well-being.
Key Responsibilities
• Assessment:
Evaluate patients' and families' social, emotional, and physical needs to identify challenges.
• Counseling & Support:
Offer emotional support and counseling to help patients and families cope with illnesses, injuries, and the healthcare system.
• Care Planning:
Develop individualized care plans with patients and families to achieve specific health and social goals.
• Advocacy:
Act as a patient advocate, ensuring they receive appropriate and timely care and access necessary resources.
• Resource Navigation:
Link patients and families to community resources, such as food assistance, housing support, and other services.
• Team Collaboration:
Work with interdisciplinary healthcare teams (nurses, therapists) to provide holistic, patient-centered care.
• Documentation:
Maintain accurate and thorough records of patient interactions, interventions, and progress.
• Crisis Intervention:
Respond to and provide support during crisis situations.
Qualifications & Skills
• Education: A Master's degree in Social Work (MSW) from an accredited program is required.
• Licensure: Must be a Licensed Clinical Social Worker (LCSW) or hold a similar state-recognized license.
• Experience: Prior experience in home health, hospice, or palliative care is often preferred.
• Interpersonal Skills: Excellent communication, empathy, and counseling skills are essential.
• Technical Skills: Proficiency in electronic health records (EHR) is necessary.
• Problem-Solving: Strong organizational and problem-solving abilities are crucial for managing complex cases.
