Summary Job Description:
The Home Liaison will encompass aspects of the following: Ensures that all program and environmental requirements related to a Family Care home are being met and maintained. Ensures that the fiscal and other provider needs and concerns are addressed and are consistent with the needs and concerns of the individuals in the home. This position is responsible for acting as a direct link between the Family Care Provider and Little Flower Children’s Services Family Care Program.
This position is a full-time, 35 hour per week position and is hybrid remote, requiring regular travel to family care homes primarily in Queens County and adjoining counties with periodic travel for team meetings at Little Flower’s Wading River office. The role may require travel to other locations to address client issues and administrative tasks as needed to ensure management of services provided in Queens, Brooklyn, Suffolk and Nassau counties, as well as other business-related locations when needed. From time to time, in emergencies, this position requires work outside of normally scheduled working hours.

Our staff of more than 500 builds well-being by providing prevention services, foster care, residential treatment care, adoption services, programs and services for individuals with intellectual and developmental disabilities, and medical and mental health services.
Principal Responsibilities
- Develop a Residential Habilitation Plan with the Family Care Provider and the individual which is consistent with the individual’s “Life Plan.”
- Balance the responsibility of ensuring the safety of the individuals served while working with the Family Care Provider to resolve issues/concerns that will enable the provider to meet the needs of the individual.
- Establish and maintain a good working relationship with all individuals’ Care Managers, nurses, Behavior Intervention Specialist (BIS) and all team members, both within and external to Little Flower
- Regularly ensure the Provider’s client files are complete and up to date, including but not limited to, their Residential Habilitation billing documentation, financials , Medication Administration Records, doctor’s office visit forms, and other required documentation as needed.
- Ensure that all program and environmental requirements related to a Family Care home are being met and maintained. Ensure that the fiscal and other provider needs and concerns are addressed and consistent with the needs and concerns of the individuals in the home. Complete a DDP2 for each individual on at least a bi-yearly basis or when significant changes occur to ensure the Difficulty of Care (DOC) payment rate remains accurate.
- Attend all annual and semi-annual Life Plan meetings with Care Managers; review and update Res-Hab Plan, as necessary.
- Be available and respond to emergencies during and outside of office hours.
- Track client movement on Little Flower’s MOP (Movement of Population) system whenever clients are out of the home overnight. Submit billing documentation in a timely manner after ensuring the goals match those on the current Res. Hab. Plan/Life Plan as well as to ensure the billable days coincide with the MOP system to avoid costly billing errors.
- Regularly review and pre-audit client files to ensure accuracy and remain audit-ready.
- Understand and work effectively with diverse personalities and populations.
- Balance the responsibility of ensuring the safety of the consumers served while working with the Family Care Provider to resolve issues/concerns that will assist the provider in meeting the needs of the individual.
- Intervene when the Family Care Provider and/or consumer are in crisis.
- Must be able to evaluate the Family Care Provider’s skills and abilities. This critical evaluation will determine the ongoing certification of the Family Care Provider.
- Conduct and document monthly home visits. This documentation is to include a walkthrough of the home and review of the monthly fire drill, as well as Personal Allowance Ledgers.
- Confirm that program and environmental requirements are met and maintained.
- Develop a Residential Habilitation Plan of Care with the Family Care Provider and the Consumer which is consistent with the individual’s Life Plan.
Required Qualifications and Experience
- Bachelor’s Degree and one year of experience required
- Valid NYS Driver’s license
Travel Requirements
This position offers a full time, hybrid-remote schedule requiring substantial weekly travel to client homes in NYC and Long Island, primarily in Queens County, and periodic in-person staff meetings in Wading River.
