Position Summary
- Reports to the Navigation Director
- Collaborates with Physicians, APPs, Scheduling Teams, Front End Teams, and Clinical Support Teams
- Supervises: None
- Required: Active Medical Assistant (MA) certification and at least 2 years of experience in a clinical or specialty care setting
- Preferred: LVN or RN licensure
- Preferred: Previous experience in urology or other longitudinal care specialties
- Preferred: Strong knowledge of urologic conditions, procedures, and patient education
- Proficiency in electronic medical records (EMR) and patient tracking software
- Excellent interpersonal, organizational, and time management skills
- Serve as the primary point of contact for patients with multi-step or complex care plans
- Provide clear, compassionate guidance on upcoming appointments, procedures, and treatment milestones
- Conduct timely outreach to patients at designated intervals, reinforcing adherence and addressing questions
- Identify and document barriers to care, and escalate issues to appropriate team members when needed
- Track patient progress along clinical pathways and ensure timely coordination of services (labs, imaging, referrals, procedures)
- Collaborate closely with providers, front office staff, and clinical teams to ensure care moves forward as ordered
- Accurately document all patient communication, care plan updates, and task completion in appropriate systems
- Reinforce provider instructions and offer education tailored to each patient’s treatment plan
- Monitor patient records to flag delays or missed steps, proactively resolving any gaps
- Participate in ongoing discussions to refine pathway protocols and support workflow improvement
- Promote a positive patient experience through timely, respectful, and professional communication
- Maintain strict confidentiality and comply with all HIPAA and clinic policies
- Perform additional navigation or support tasks as assigned
Physical Demands
- Regular walking, sitting, typing, reaching, and light lifting up to 40 pounds
- Visual acuity and auditory ability to assess patient needs and input data
- Occasional physical strain from multitasking or high-volume communication
- In-office or Hybrid-remote options available.

- Pay Range: $26.00 – $42.00 per hour, based on licensure and relevant experience
