Hours: 7:30 a.m. - 3:30 p.m.
We are looking for a compassionate, patient, and motivated Personal Community and Employment Professional to work one-on-one with a 21-year-old woman. In this role, you’ll provide personalized support at her day program and in the community helping her explore activities, build meaningful relationships, and achieve her personal goals.
This position is perfect for someone who enjoys helping others grow, values connection, and is excited to make a real difference in someone’s life while also assisting with goals and daily supports.

She is a caring, fun-loving, and social young woman who enjoys small groups, music, and laughter. Her favorite activities include:
Singing and playing the keyboard
Going outside and enjoying the breeze

Watching Frozen and Pocahontas
Relaxing in her recliner
Eating pudding
Spending time with people who are patient and kind
She thrives when supported by someone who communicates clearly, encourages her with positive praise, and helps her stay engaged in activities she loves while also introducing her to new opportunities.
Key Responsibilities
Personalized Support:
Provide one-on-one assistance at the day program and in the community
Assist with personal care, G-tube water and medication, and other health-related needs
Be patient when communicating and allow extra processing time
Offer encouragement, motivation, and positive reinforcement
Community Engagement:
Support participation in meaningful community activities and events
Assist with exploring new interests
Develop and follow a personalized schedule based on her preferences and goals
Build and maintain positive relationships with community members
Planning & Documentation:
Help create and update personalized plans based on her interests and needs
Track progress toward goals and outcomes
Plan activities that expand social skills, community involvement, and personal growth

Must be able to complete Med Tech Certification
Minimum Qualifications:
High school diploma or GED
Valid driver’s license with at least 2 years of driving experience
Clean driving record
Ability to lift 50 lbs and assist with physical support as needed
