What you’ll do
Deliver skilled PT visits in patient homes
Build and update care plans with fast approvals
Document quickly using modern EMR (no outdated tools)
Track outcomes that matter—mobility, independence, fewer readmissions
Partner with nurses, aides, and families for full care support
Take ownership of your caseload, day one
Must-haves
Active PT license in Missouri
1+ year home health or outpatient PT
Valid driver’s license and reliable car
Able to manage 5–6 visits/day
Work authorized in US
