About the Role:
Customized Living Services follows a person-centered approach, valuing the uniqueness of each individual through our philosophy, policies, and procedures. Together with Wilder’s Healthy Aging & Caregiving Services, you will join a collaborative team that supports the health and independence of adults as they age and receive care in the community, which is critical to Wilder’s mission of improving lives today and for generations to come. As a valued team member, you will provide person-centered care for older adults, adults with disabilities, and also those with chronic conditions. Support clients with activities of daily living assistance such; as medication management, personal cares, housekeeping, and socialization.
Qualifications:
Associates degree required (Bachelor’s Degree Preferred). At least one year of home health nursing or geriatric nursing experience required. LPN Licensure – State of MN & LPN Program Certificate. Valid Driver’s License required.
