Admissions / Discharge Coordinator – Chief First Impression Officer

Think of this role as the ultimate mix of tour guide, matchmaker, and paperwork wizard. As our Admissions / Discharge Coordinator, you’ll be the smiling face (and calming voice) families meet when they’re deciding where their loved ones will feel most at home. You’ll also be the behind-the-scenes pro making sure all the T’s are crossed, I’s are dotted, and that everyone feels welcome from day one to discharge day.


What You’ll Do (a.k.a. Your Superpowers):

  • Keep our admissions process smoother than a fresh jar of peanut butter—writing, updating, and recommending changes to policies and procedures.
  • Channel your inner ambassador by sharing our facility’s value with the community (bonus points if you can make people laugh along the way).
  • Work your networking magic by cultivating referrals and building solid relationships with hospital discharge planners and other referral sources.
  • Gather and verify applicant info like a detective—applications, medical records, family chats—you’ll piece it all together with kindness and accuracy.
  • Lead engaging tours of our facility (think: less realtor, more welcoming committee).
  • Collaborate with our team to find just the right placement when multiple beds are available.
  • Be a steady guide for patients and families navigating admissions and discharges—making the process a little less stressful and a lot more human.

Requirements

What We’re Looking For (a.k.a. The Secret Sauce):

  • A Licensed Practical Nurse (LPN) license—because clinical know-how takes this role from good to great.
  • At least one (1) year of experience in healthcare admissions—or a very convincing ability to fake it until you make it (kidding… but seriously, experience required).
  • A strong respect for Residents’ Rights and our Facility Policies—because we’re big fans of following the rules around here.
  • Eagle-eye detail skills and customer service that could rival a 5-star hotel concierge.
  • Friendly, responsible, dependable, and team-oriented—translation: someone we actually want to work with on a Monday morning.
  • A natural communicator and motivator—if you can charm people, keep things organized, and juggle multiple tasks without breaking a sweat, we want you.
