CASE MANAGER

Job Summary: The Case Manager monitors, directs, and assists other unit staff to assure that care is delivered according to each resident’s care plan, the Medical Plan of Care, and State/Federal regulations. The Case Manager serves as the nursing liaison for residents, families, physicians, interdisciplinary team members and outside service providers.

Job Relationships:
A. Reports to: Director of Nursing and Assistant Director of Nursing.
B. Supervises: Monitors and directs staff RN’s, LPN’s, TMA’s and CAN’s to provide the highest quality resident care.
C. Internal Relationships: Coordinates resident care with interdisciplinary team members and other professional providers.
D. External Contacts: Resident family members, physicians, and other service providers.

Hours of Work: 40 hours per week, Monday through Friday and required weekend and holiday rotation.

Dress code: Casual dress clothes or scrub uniform and Identification badge.

Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity

1. Education:
    a. Graduate of an accredited school of nursing.
    b. Must be a Registered Nurse in MN
    c. CPR certified
2. Experience
    a. Minimum of two-years long-term care experience preferred.
3. Job Knowledge
    a. Working knowledge of Medicare, State/Federal regulations, and Case Mix.
    b. Experience in completing MDS’s and the MDS process
    c. Skilled in work organization, time management, and delegation
    d. Possess and use a resident focus and service excellence philosophy.
    e. Possess a professional, courteous, and calm manner
    f. Have good communication skills in the English language

Physical Demands of the job:
1. Ability to work under moderate levels of stress
2. Requires frequent change of position to include: standing, sitting, walking, bending, and kneeling
3. Patient handlers must sign & comply with the Skilled Nursing Facility Safe Patient Handling Policy.

Mental Demands of the Job:
1. Judgement of Initiative: A wide variety of complex decisions are made. Initiative can be measured by timely accomplishments of assigned projects and predetermined goals.
2. Independent Action: A high degree of independent action necessary. The job requires self-motivation to function at full productivity.
3. Effect of Error- errors can result in poor resident care, budge discrepancies, staff problems, and unsafe environment.
4. Contact with others: Position requires contact with virtually all persons associated with the nursing home.

Principle Job Responsibilities:
Responsibility: Maintains the assigned unit to meet State and Federal regulations and facility standards:
Tasks:
A. Promotes and implements policy & procedures under the direction of the DON/ADON that assist the resident’s in achieving their highest practicable physical, psychosocial, and spiritual function.
B. Directs, monitors, and assists unit staff to assure residents experience no preventable decline in function.
C. Monitors the quality of resident care on the unit and provides direction to unit staff to improve or maintain the highest quality care.
D. Serves as a resource person to unit staff in the areas of resident care, clinical practice, regulations, and facility policy.
E. Initiates disciplinary action when appropriate for unit staff and reports to DON/ADON
F. Assist’s LPN’s or staff RN’s with disciplinary action as needed.
G. Assis with quality assurance of care as defined by the DON/ADON and Medical Director
H. Keeps DON/ADON current on any unusual or critical situations with residents, family and staff.
I. Makes recommendations to DON, ADON, or MDS Coordinator for improvement of care, needed supplies and equipment
J. Assists DON/ADON with performance evaluations of staff on the unit.
K. Participates in continuing education programs both inside and out of the facility
L. Other duties as assigned by DON/ADON

Responsibility: Perform duties of RN House Supervisor position.
Tasks:
A. Performs physician rounds with resident’s attending physicians.
B. Communicates any changes in or concerns about a resident’s condition to the physician and seeks appropriate interventions
C. Transcribes physician orders or co-signs the orders transcribed by RN or LPN.
D. Performs appropriate clinical assessments for residents who are acutely ill and documents the findings of these assessments in the resident record.
E. Performs and documents on-going clinical assessments as required for unit residents i.e. weight loss, condition changes, dehydration etc.
F. Makes any needed physician calls during the shift regarding resident conditions or concerns if charge nurse is busy.
G. Assist with resident admission and readmission per facility policy with the assistance of House Supervisor. Complete all necessary assessments require by an RN.
H. Performs or monitors performance of any clinically complex treatments.
I. Assists residents, families, and unit staff in problem solving related resident needs.

Responsibility: Understanding and completion of the MDS process.
Tasks:
A. Updates the plan of care or initiates/updates short-term care plans as appropriate and follows recommendations from MDS Coordinator.
B. Attends care conferences for residents on their unit.
C. Completes MDS’s accurately and in a timely manner.
D. Attends facility meetings as assigned.
Responsibility: Pain Management
Tasks:
A. Provides appropriate and attentive pain management which includes:
    a. Accepts the resident’s report of pain
    b. Relieves the physical and psychosocial symptoms associated with pain while optimizing individual and family functioning.
    c. Promotes active participation by residents and their families in selecting pain management strategies and in communicating new or unrelieved pain.
    d. Provides consistent state of the art care through multi-disciplinary care team and universal guidelines for pain management.
    e. Educates residents, families, and caregivers about pain management and continually reinforces the development and application of knowledge.

Responsibility: Continuous Quality Improvement
Tasks:
A. Demonstrates sensitivity to customers and their needs.
B. Interacts appropriately with internal customers, i.e. co-workers with department and staff across departments.
C. Interacts appropriately with external customers, i.e. patients, residents, families, medical staff, and venders
D. Role models positive behaviors

Responsibility: Resident Rights
Tasks:
A. Acts as an advocate for Resident Bill of Rights and monitors and directs unity staff in this area.
B. Assures that all residents, families, and staff are aware of resident rights.
C. Assists in the identification of vulnerable adult issues and reports the DON, ADON, Administrator, or Social Services in a timely manner.
D. Respects resident property
E. Maintains dignity and privacy of residents by knocking before entering resident room, addressing them in a respectful manner, discussing their status in a private manner, treating them in a courteous and polite manner.
