Staff Pharmacist -Full Time- Overnight

In this role, you will review and interpret medication orders by physicians to other authorized healthcare providers. Verifies orders for completeness, accuracy and appropriateness in accordance with pharmaceutical care standards and prescribed policies and procedures. Reviews patient’s profiles and problems, i.e., Drug interactions, drug allergies, therapeutic duplication, contraindications, drug-drug incompatibilities, dosing and adverse drug reactions relating to the new order or order changes. Maintains effective communication with physicians and nurses to ensure safe and appropriate use of drugs. Prepares or supervises the preparation and dispensing of medication to inpatients and outpatients. Including unit dose dispensing, IV admixture, chemotherapy, TPN, investigational drugs, other sterile products, and extemporaneous preparation, in accordance with hospital policy. Completes written records of dispensing all medication including controlled substances and investigational drugs as required by policy and regulation. Advises and consults with patient and families on medication use, dangers, side effects and composition. Maintains responsibility for resolving medication dispensing system problems for one or more patients care areas. Provides in-service education and/or orientation concerning drug use to health professionals when necessary or as appropriate. Attends and participates in meetings held on patient care units as appropriate. Routinely visits Nursing units, review patient charts, physician orders, medication records and storage areas. Conducts monthly inspections of patient care areas in accordance with Joint Commission and N.Y.S. Department of Health requirements. Reviews inspection results and quality improvement information with patient care director or department head and recommends changes as required. Routinely participates in target drug monitoring programs and pharmacy and hospital-sponsored drug therapy intervention programs. As needed, participate in quality assurance, drug utilization review, and adverse drug reaction reporting. Maintains knowledge of field by participation in continuing education programs in accordance with New York State requirements, Joint Commission standards and as necessary to perform the job effectively.
This position is a Full Time evening shift 2:45pm-11:00pm am with rotating weekdays/weekends/holidays.
Required Criteria
Degree or major in Pharmacy.
Knowledge normally acquired through completion of a bachelor’s degree plus up to one year of post-degree course work and/or an internship and less than a year of previous experience
Licensure as a Pharmacist by the State of New York
