Job Summary:
This role is ideal for a PTA who is passionate about working with seniors and thrives in a collaborative environment that bridges clinical care, social engagement, and overall well-being As the PTA - Senior Living Navigator you will have consistent presence at primarily two Senior Living Communities where you will facilitate activities that help support resident health, engage and build relationships with residents, and provide home health therapy visits within the community they call home.

Key Responsibilities:
- Deliver prescribed Home Health physical therapy interventions to residents in accordance with individualized care plans.
- Assist in developing and modifying treatment plans to ensure the best possible outcomes in coordination with the Physical Therapist.
- Document resident progress accurately and timely in compliance with regulatory and company standards.
- Promote and participate in resident wellness and engagement activities that support mobility, function, and social interaction.
- Coordinate scheduled health-related events for residents and maintain consistent activities that ensure resident engagement and positive experiences.
- Communicate and collaborate regularly with Reliant at Home team members and Senior Community personnel to ensure continuity of care.
- Educate residents, families, and staff on mobility strategies, injury prevention, and safe practices.
- Foster a supportive, respectful environment that encourages resident participation.
Essential Functions:
- Administers the physical therapy treatment plan as developed by the Physical Therapist.
- Community/Resident Engagement Activities (Fall/Risk Assessments, Walking Club, Education/Exercises, Building Relationship with Community Stakeholders (Executive Directors, Activity Directors, etc.)
- Apply concepts of infection control and universal precautions in performing patient care activities.
- Accepts clinical assignments that are consistent with education and competence to care for patients.
- Uses effective interpersonal relations and communication skills.
- Meets mandatory continuing education requirements.
- Promotes compliance with all fiscal intermediaries and/or other third-party payers, though education, coaching, and other assistance as necessary.
- Demonstrates commitment, professional growth, and competency.
Qualifications
- Minimum of 1 years’ experience in Home Health.
- Homecare Homebase a plus.
- Part B experience a plus.
