Essential duties/responsibilities (This is not an exhaustive list; Other duties may be assigned):

  • Deliver case management services as listed in OAR 411-415-0050 Standards for Case Management Services to individuals with developmental disabilities.
  • Act as a mandatory abuse reporter. Ensure all reported serious events related to an individual are documented using the Lifeways Incident Report form, per Malheur County CDDP/Lifeways policies.
  • Assist program manager in monitoring the quality of services delivered within the county, and in the identification of existing and insufficient service delivery resources or options.
  • Provide information and timely referral for individuals and their families regarding developmental disability services available within Malheur County and assists them to access services and resources.
  • Maintain an assigned caseload and provide case management services which may include problem identification, goal setting, progress notes, annual plans and summaries, and monitoring individual services.
  • Coordinate with other State, public and private agencies regarding services to individuals with development disabilities.
  • Attend all client service meetings as necessary.
  • In consultation with client’s team members provide crisis services by assessing, identifying, planning and monitoring client needs in coordination with the crisis provider and regional crisis coordinator.
  • Serve as an advocate/liaison in assisting clients to obtain necessary services, which may include transitioning, community integration, resource and referral.
  • May provide or facilitate transportation to service appointments.
  • Provide case management and support for clients placed in nursing homes, group homes, foster homes, private homes, or supported living situations and other residential/vocational day services.

Qualifications


Required:

  • Bachelor’s degree in social science, behavioral science, or closely related field
  • OR a bachelor’s degree in any field and one year of human services related experience
  • OR an Associate degree in behavioral science, social science, or a closely related field and two years of human services related experience
  • OR a High School Diploma and three years of human services related experience
  • Must obtain Translation certification within 6 months of hire.
  • Must have knowledge of the public service system for developmental disabilities services in Oregon
  • Valid driver’s license
  • Successful completion of background check
  • Ability to read, speak, and write in both English/Spanish in a business setting
