Hospice Outreach Coordinator

Summary/Objective
The Hospice Outreach Coordinator serves as a strategic partner to the Sales team in the Kansas City metropolitan area, focused on expanding hospice awareness and facilitating early identification of hospice-appropriate patients. Unlike the Clinical Hospice Liaison, this non-clinical role centers on educational outreach, provider engagement, and in-service delivery to healthcare facilities, physician offices, and community organizations. The Outreach Coordinator helps open doors for admissions by equipping providers with the knowledge and confidence to refer patients earlier in their care journey.
Essential Functions
Outreach & Relationship Building
- Establish and maintain strong relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices.
- Serve as a trusted resource for hospice education, bridging gaps in provider understanding and referral readiness.
- Collaborate with Hospice Liaison/Account Managers to support strategic account growth and referral development.
Education & Inservice Delivery
- Plan and conduct regular in-services tailored to the needs of healthcare professionals, including physicians, nurses, social workers, and case managers.
- Present on hospice philosophy, eligibility criteria, benefits, and common misconceptions.
- Customize educational materials to reflect the unique needs of each account and audience.
Early Identification Support
- Partner with clinical and sales teams to help providers recognize hospice-appropriate patients earlier in the disease trajectory.
- Facilitate discussions around care goals, transitions, and hospice readiness.
- Share insights from outreach efforts to inform account strategy and messaging.
Sales Team Collaboration
- Work closely with Hospice Liaison/Account Managers to align outreach efforts with referral goals.
- Support account onboarding through educational engagement and resource sharing.
- Participate in strategic planning and territory reviews to identify opportunities for growth.
Community Engagement & Marketing
- Represent the hospice program at community events, health fairs, and professional seminars.
- Support marketing initiatives by promoting hospice awareness and distributing educational materials.
- Help expand referral networks through consistent and compassionate outreach.
Qualifications
- Master’s degree in Gerontology, Public Health, Social Work, or related field preferred.
- Bachelor’s degree in Family Studies, Human Services, or related discipline required.
- Experience in healthcare outreach, education, or community engagement strongly preferred.
- Excellent presentation, communication, and interpersonal skills.
- Ability to work independently and collaboratively in a dynamic, mission-driven environment.
Supervisory responsibilities: This position does not involve any direct supervisory responsibilities.
Budgetary/Fiscal Responsibility: Responsible for using resources wisely to support cost-effective operations.

Maintains confidentiality of privileged information and supports the employee’s and patient’s right to privacy.
Work Environment/Schedule/Travel: Home office based. Monday through Friday. Travel to program sites as directed.
